How do I register for CNESST’s “Safe Motherhood” program ?
These are the steps involved in registering for the program :
You must first come to the clinic to complete the form. You’ll need to bring your health insurance card, your social insurance number, your employer’s full address and the name and phone number of your supervisor.
You must then confirm your pregnancy with a blood test. The form will be handed to you when you come to fill in the form.
The result of this blood test is verified by our team (every Tuesday and Thursday).
Once the pregnancy has been confirmed by a blood test, the form is sent to the CNESST for an assessment of your claim.
You will then be contacted upon receipt of the reply. For patients working in the Montérégie region, the delay is usually 48 hours. For patients working in the Montreal area, the delay is more than a week.
Once we’ve received your evaluation response, we will contact you to arrange for you to pick up your form. You will receive one copy (WHITE) for you and one copy (GREEN) for your employer. The YELLOW copy must be forwarded BY YOU to the CNESST to open your file (Fax: 855-722-8081). Please note that the CNESST recommendations are attached. We strongly suggest that YOU make a copy for yourself.
